Complete a Screening for 2023

A biometric screening is a good way to check in on your health status. If you missed participating in an on-site screening, there’s still time, as the last one is March 30.

If the on-site screenings don’t fit your schedule or preferences, there are three other ways to complete a biometric screening and earn PATH points:

  • CVS MinuteClinic — Download and print a CVS MinuteClinic Voucher from your PATH account by going to Benefits, CVS Health. Make an appointment at a CVS MinuteClinic location near you and take the voucher with you. You will not receive an A1c result unless you are diabetic.
  • LabCorp — Download or email a LabCorp Test Requisition Form from your PATH account by going to Benefits, LabCorp and then LabCorp Patient Service Center. Make an appointment at a location near you and take your form with you.
  • Primary Care Physician — Download and print a Physician Form from your PATH account by going to Benefits, LabCorp and Physician Form. Make an appointment to see your Primary Care Provider and bring the form with you. Once your physician completes and signs the form, fax it to LabCorp or upload it to your PATH account.

You earn 5,000 points for completing a screening — no matter which option you choose — and receive the points via a voucher code. So no matter when you complete the screening, you can redeem the points any time throughout the year to add to any quarterly earnings! The voucher code arrives via email.

There’s also an extra $25 cash incentive when you complete a screening, which is above and beyond your voucher-code points and in addition to the annual $400 maximum PATH incentive earnings.
