Transform for Prediabetes Helps Prevent or Delay Type 2 Diabetes

PATH’s Transform for Prediabetes program is back this year, helping to prevent or delay the onset of Type 2 diabetes in participants. You can complete a survey to determine your eligibility by going to the Benefits Page and clicking VP Transform for Prediabetes. You’ll also receive emails throughout 2023 with a link to click to access the survey.

Cherry-topped tartPrediabetes is a serious health condition where blood sugar levels are higher than normal, but not yet high enough for a Type 2 diabetes diagnosis, according to the Centers for Disease Control and Prevention. Approximately 88 million American adults — or more than 1 in 3 — have prediabetes — and more than 84 percent do not know they have it.

If you have prediabetes, the cells in your body do not respond to insulin, a hormone made by your pancreas, as they should. Insulin is a “key” that unlocks cells to let in blood sugar the cells use as energy. When your cells do not respond correctly, your pancreas makes more and more insulin to try to get the job done. Eventually, though, your pancreas just can’t keep up, and your blood sugar rises, setting the stage for prediabetes.

You can have undetected prediabetes for years with no clear symptoms, until serious health problems such as Type 2 diabetes, or worse, develop. But you can delay or prevent prediabetes by taking the right steps now — like completing the Transform for Prediabetes survey!

  • Regular check-ins with a health coach.
  • Digital body-weight scale (since there is a goal of a 5–7 percent reduction of starting body weight).
  • Fitbit Tracker to monitor physical activity (with a goal of 150 minutes per week).
  • Access to a separate Transform App that includes helpful tools like meal tracking (by taking pictures) and weekly lessons.
