• Download the voucher.
• Make an appointment at a CVS location near you at a MinuteClinic (www.minuteclinic.com).
• Bring the voucher to your appointment.

• Go to the Benefits Page on PATH and click LabCorp.
• Click Start Now.
• Under LabCorp, click Download Form.
• Find the nearest LabCorp location and make an appointment. (www.labcorp.com/findalab).
• Bring the voucher to your appointment.

• Physician visit
• Make an appointment with your Primary Care Physician.
• Go to the Benefits Page on PATH and click LabCorp.
• Under Physician Form, click Download Form and bring it to your appointment.
• Once your physician fills out the form, upload it to your PATH account from where you downloaded it, or fax it to LabCorp at 888-972-1871.
